Gifted Smiles

Sorry for any inconvenience, the Gifted Smiles application portal and process have been temporarily placed on hold.  Our goal is to have the application portal open again this spring.  If you have already started the application process for Gifted Smiles, you will be contacted this spring.  Thank you in advance for your patience.

General Eligibility Requirements

  • Your child is 18 years old or younger.
  • Your total family income is 200% of the poverty level or less. (See chart below.)
  • Your child receives regular dental care and has good oral hygiene.
  • Your child is not currently in orthodontic treatment or in between phases of treatment.
  • Your child has not received previous orthodontic treatment.
